INSTRUCTIONS:
1. Uninstall the YouTube app
2. Create directory : /system/app/YouTube with 755 permission
3. Put the downloaded apk to /system/app/YouTube.apk
4. Reboot

What am I doing wrong? I've done it before with the older ones and it worked. This one still has ads for me. I followed the instructions and I redownload it again but still not ad free. What should I try doing?

This is for the arm version by the way.

Step 1 in the instructions is generic and vague... Most certainly you have 2 youtube apps, one is come with rom and the other is updated one from play store. Go to play store ->youtube and uninstall there. It will ask you if you want to revert to system app, select yes then delete youtube.apk in system/app/youtube folder and replace with ad free one.... and continue to step 2, 3, 4... etc
Edit: BTW, permission for youtube.apk is 644. Reboot and profit

I don't have 2 YouTube app, buddy. I deleted the originally from the root long time ago and use the ad free one.Now when I delete the one I had on root (the old ad free one) for the new one and it shows me ad. I cleared cache already. I don't know, guess I'll just stay with this. :(

Looks like I'm not the only one. Works for some but not everyone.

Edit: I found out the issue! If you're logged in on youtube, sign out! That's how it stop showing ads for me.
